By the way, something very similar to this happened in (if memory serves) Australia around a year or so ago. Some lady served beef Wellington with poison mushrooms to her in-laws...
She did, although not the same mushroom. Common ink caps are only poisonous with alcohol. She served death caps, which also act more quickly. I - came up with the idea when I read an article about ink caps in a magazine, and the Australian case was a co-incidence.
